class=\"wp-block-spacer\"><\/div>\n\n\n\n<h4 class=\"wp-block-heading has-heading-5-font-size\" id=\"h-background\"><br><span class=\"fw-600\"><mark style=\"background-color:rgba(0, 0, 0, 0)\" class=\"has-inline-color has-ocean-blue-color\">Background<\/mark><\/span><\/h4>\n\n\n\n<div style=\"height:12px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>Inflammatory signals in IgA nephropathy are thought to play a role in glomerular injury, podocyte loss, and mesangial activation contributing to glomerulosclerosis and interstitial fibrosis.<sup>1,2<\/sup><\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>The renin-angiotensin system (RAS) is thought to play a role in the development of glomerulonephritis,<sup>3<\/sup> providing the rationale for the use of angiotensin-converting enzyme inhibitors (ACEi) or angiotensin receptor blockers (ARB) in IgA nephropathy.<sup>1<\/sup> Although considered first-line therapy for patients with IgA nephropathy at risk of progressive kidney injury, chronic kidney disease (CKD) is slowed but not prevented.<sup>1<\/sup> There is an urgent need for novel IgA nephropathy treatments.<sup>1<\/sup><\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p><span style=\"text-decoration: underline;\"><a href=\"https:\/\/clleancode.top\/travere-redesign\/episode\/role-of-endothelin-in-igan\/\">Endothelin type A receptor (ET<sub>A<\/sub>R) antagonism<\/a><\/span> is thought to be a novel approach to treating IgA nephropathy, as increased endothelin-1 (ET-1) levels, along with angiotensin II (Ang II), have been observed in IgA nephropathy.<sup>1,4,5 <\/sup>Clinical evidence suggests antiproteinuric and nephroprotective effects of ET<sub>A<\/sub>R and RAS inhibition.<sup>6<\/sup><\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>Sparsentan, an orally active, <span style=\"text-decoration: underline;\"><a href=\"https:\/\/clleancode.top\/travere-redesign\/publications\/protective-mechanism-sparsentan-chronic-kidney-disease-models\/\">single-molecule dual endothelin angiotensin receptor antagonist<\/a><\/span>, has demonstrated <span style=\"text-decoration: underline;\"><a href=\"https:\/\/clleancode.top\/travere-redesign\/publications\/efficacy-and-safety-of-sparsentan-versus-irbesartan-in-patients-with-iga-nephropathy-protect-2-year-results-from-a-randomised-active-controlled-phase-3-trial-2\/\">superior preservation of kidney function and antiproteinuric effects<\/a><\/span> in clinical studies.<sup>7,8<\/sup> However, more research is needed in the mechanism of nephroprotection of sparsentan.<sup>1<\/sup><\/p>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h5 class=\"wp-block-heading\" id=\"h-aim\"><br><mark style=\"background-color:rgba(0, 0, 0, 0)\" class=\"has-inline-color has-ocean-blue-color\"><span class=\"fw-600\">Aim<\/span><\/mark><\/h5>\n\n\n\n<div style=\"height:12px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>The objective of this study was to assess the mechanisms of nephroprotection of sparsentan versus that of standard of care, losartan, in an experimental IgA nephropathy mouse model.<sup>1<\/sup><\/p>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h5 class=\"wp-block-heading\" id=\"h-approach\"><br><mark style=\"background-color:rgba(0, 0, 0, 0)\" class=\"has-inline-color has-ocean-blue-color\"><span class=\"fw-600\">Approach<\/span><\/mark><\/h5>\n\n\n\n<div style=\"height:12px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>Four-week-old gddY mice, an IgA nephropathy mouse model, were treated with either sparsentan or losartan for 8 or 16 weeks.<sup>1<\/sup> <\/p>\n\n\n\n<p>At Week 12 or Week 20, kidney sections was excised from the mice.<sup>1<\/sup> RNA was extracted from the kidneys and analyzed.<sup>1<\/sup><\/p>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h5 class=\"wp-block-heading\" id=\"h-findings\"><br><mark style=\"background-color:rgba(0, 0, 0, 0)\" class=\"has-inline-color has-ocean-blue-color\"><span class=\"fw-600\">Findings<\/span><\/mark><\/h5>\n\n\n\n<div style=\"height:12px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p><span class=\"fw-600\">In the gddY IgA nephropathy mouse model, sparsentan lowered the albumin-creatinine ratio (ACR) more rapidly and to a greater extent than losartan.<sup>1<\/sup><\/span><\/p>\n\n\n\n<div style=\"height:12px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>Baseline ACR was significantly different for the sparsentan group compared to the control and losartan group (<em>P<\/em>&lt;0.01).<sup>1<\/sup> This was likely due to the variability of disease onset in gddY mice.<sup>1<\/sup><\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>ACR across all treated mice decreased compared to the control group, with sparsentan demonstrating a more rapid decrease and to a greater extent.<sup>1<\/sup> Given the differences in baseline ACR, additional statistical analyses were conducted.<sup>1<\/sup> Findings supported the rapid decrease in ACR with sparsentan (<em>P<\/em>&lt;0.0001).<sup>1<\/sup><\/p>\n\n\n\n<div style=\"height:40px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p><span class=\"fw-600\">Sparsentan protected against development of glomerulosclerosis, podocyte injury, and loss of glomerular glycocalyx.<sup>1<\/sup><\/span><\/p>\n\n\n\n<div style=\"height:12px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<div class=\"wp-block-group is-layout-constrained wp-block-group-is-layout-constrained\" style=\"padding-left:calc(64px * var(--scale))\">\n<p><span class=\"fw-600\">Glomerulosclerosis<\/span><\/p>\n\n\n\n<div style=\"height:11px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>Sparsentan significantly attenuated the development of glomerulosclerosis compared to losartan (<em>P<\/em>&lt;0.001).<sup>1<\/sup><\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p><span class=\"fw-600\">Podocyte injury<\/span><\/p>\n\n\n\n<div style=\"height:12px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>Both sparsentan and losartan significantly prevented the loss of podocytes (<em>P<\/em>&lt;0.01 and <em>P<\/em>&lt;0.05, respectively).<sup>1<\/sup> However, sparsentan demonstrated greater preservation of Wilms tumor (WT)-1-positive cells comparable to the control group.<sup>1<\/sup><\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p><span class=\"fw-600\">Loss of glomerular glycocalyx<\/span><\/p>\n\n\n\n<div style=\"height:12px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>Sparsentan also resulted in significant preservation of glycocalyx versus losartan (<em>P<\/em>&lt;0.001). <sup>1<\/sup><\/p>\n<\/div>\n\n\n\n<div style=\"height:40px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p><span class=\"fw-600\">Sparsentan prevented the upregulation mRNA expression of ET-1, ET<sub>A<\/sub>R and angiotensin subtype 1 receptor (AT<sub>1<\/sub>R), and proinflammatory genes.<sup>1<\/sup><\/span><\/p>\n\n\n\n<div style=\"height:12px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>ET-1 and ET<sub>A<\/sub>R were strongly upregulated in gddY control mice (<em>P<\/em>&lt;0.05), and the increase of AT<sub>1<\/sub>R was significant as well (<em>P<\/em>&lt;0.05).<sup>1<\/sup> Although expression remained high, gene expression was attenuated with sparsentan and losartan (<em>P<\/em>&lt;0.01).<sup>1<\/sup><\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>Increased mRNA expression of proinflammatory genes was significantly attenuated with sparsentan and losartan (<em>P<\/em>&lt;0.05).<sup>1<\/sup><\/p>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h5
