style=\"height:12px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>Primary IgA nephropathy is the most common glomerular disease worldwide and a leading cause of kidney failure.<sup>2,3<\/sup> It is characterized by the deposition of pathogenic immune complexes in the glomerular mesangium, which leads to the release of inflammatory cytokines and complement activation.<sup>2-4 <\/sup>The pathology of IgA nephropathy is diverse and can include<sup>5<\/sup>: <\/p>\n\n\n\n<div style=\"height:10px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n<div class=\"wp-block-list-wrapper\">\n<ul class=\"wp-block-list\">\n<li>Variable glomerular injury<\/li>\n\n\n\n<li>Tubulointerstitial fibrosis<\/li>\n<\/ul>\n<\/div>\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>In IgA nephropathy, elevated proteinuria has been demonstrated as the strongest clinical predictor of disease progression to kidney failure.<sup>6,7<\/sup> Studies suggest that 50% of adults have already progressed to stage 3 chronic kidney disease (CKD) at diagnosis.<sup>3,8<\/sup> <\/p>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h5 class=\"wp-block-heading\" id=\"h-aim\"><br><mark style=\"background-color:rgba(0, 0, 0, 0)\" class=\"has-inline-color has-ocean-blue-color\"><span class=\"fw-600\">Aim<\/span><\/mark><\/h5>\n\n\n\n<div style=\"height:12px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>The objective of this study was to characterize disease stage and histological patterns of IgA nephropathy at diagnosis.<sup>1<\/sup><\/p>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h5 class=\"wp-block-heading\" id=\"h-approach\"><br><mark style=\"background-color:rgba(0, 0, 0, 0)\" class=\"has-inline-color has-ocean-blue-color\"><span class=\"fw-600\">Approach<\/span><\/mark><\/h5>\n\n\n\n<div style=\"height:12px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>This non-interventional, retrospective, large US cohort study evaluated clinical characteristics of IgA nephropathy and calculated MEST-C scores using diagnostic biopsy samples.<sup>1<\/sup> Adult patients with IgA nephropathy, as diagnosed by a kidney biopsy, without a history of kidney transplants were included.<sup>1<\/sup><\/p>\n\n\n\n<hr class=\"wp-block-separator has-alpha-channel-opacity\"\/>\n\n\n\n<div style=\"height:15px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h5 class=\"wp-block-heading\" id=\"h-findings\"><br><mark style=\"background-color:rgba(0, 0, 0, 0)\" class=\"has-inline-color has-ocean-blue-color\"><span class=\"fw-600\">Findings<\/span><\/mark><\/h5>\n\n\n\n<div style=\"height:12px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p><span class=\"fw-600\">Baseline characteristics<\/span><\/p>\n\n\n\n<div style=\"height:12px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>At diagnosis, most patients already presented with proteinuria and had substantial renal injury, stage 3 CKD or higher, and high MEST-C scores for mesangial hypercellularity, segmental sclerosis, and tubular atrophy.<sup>1<\/sup><\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>The overall study population that met the eligibility criteria was 4,375 patients.<sup>1<\/sup><\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" staging<\/span><\/p>\n\n\n\n<div class=\"wp-block-group is-vertical is-layout-flex wp-container-core-group-is-layout-fe9cc265 wp-block-group-is-layout-flex\">\n<div style=\"height:5px;width:0px\" m<sup>2<\/sup> in patients with available eGFR data (87.5%; 3,828\/4,375)<sup>1<\/sup><\/li>\n\n\n\n<li>Among this subset of patients, 74.6% (2,856\/4,375) of patients had stage 3 CKD or class=\"wp-block-list-wrapper\">\n<ul class=\"wp-block-list\">\n<li>Patients with available proteinuria data (52.1%; 2,280\/4,375) had a mean value of 4.2 g\/day<sup>1<\/sup><\/li>\n\n\n\n<li>85.0% patients had mild to moderate mesangial hypercellularity<sup>1<\/sup><\/li>\n\n\n\n<li><span class=\"fw-600\">E<\/span>ndocapillary hypercellularity: 16.9% (741\/4,375) of patients had minimal style=\"height:5px;width:0px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>The association between proteinuria \u22651 g\/day and the presence of all MEST-C histological characteristics was statistically significant.<sup>1<\/sup><\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<p>Similar associations were seen between most histological characteristics and proteinuria values \u22653.5 g\/day compared with &lt;3.5 g\/day.<sup>1<\/sup><\/p>\n\n\n\n<div style=\"height:40px\" aria-hidden=\"true\" class=\"fw-600\">Associations between MEST-C scores and CKD stage<\/span><\/p>\n\n\n\n<div style=\"height:5px;width:0px\" aria-hidden=\"true\" class=\"wp-block-spacer wp-container-content-4daaf377\"><\/div>\n\n\n\n<p>The pathology of IgA nephropathy at diagnosis indicated an association between high MEST-C scores and CKD staging.<sup>1<\/sup><\/p>\n\n\n\n<div style=\"height:20px\" aria-hidden=\"true\" class=\"wp-block-spacer wp-container-content-62aae154\"><\/div>\n\n\n\n<p>The odds of mesangial hypercellularity, segmental sclerosis, tubular atrophy, and
